Why is jawbone lost at all?

The jawbone is a living tissue that is constantly being built up and broken down. As long as a tooth is anchored in the bone with its root, this area remains stable – because it is mechanically stressed by daily chewing. However, if a tooth is missing, the body recognizes this region as „no longer needed" and breaks down the bone in this area.

Within the first year after tooth loss, the jawbone can shrink by up to 25%—both in width and height. Other causes of bone loss include chronic gum inflammation (periodontitis), long-term use of ill-fitting dentures, or accidents. In all these cases, the bone may later be unable to provide sufficient support for a Dental implant offer – unless it is specifically rebuilt.

What is a bone graft?

Under the term Bone augmentation – also known medically as augmentation – refers to various dental procedures used to restore missing bone volume in the jaw. The goal is to create sufficient stable bone tissue in which a dental implant can heal firmly and securely.

Depending on the initial situation, different methods are used:

  • Small, local augmentation: Here, the gap around the implant is filled directly with bone graft material during implantation. Frequently applicable for minor bone defects.
  • External Augmentation (Lateral Augmentation): The bone is widened laterally to create space for the implant.
  • Vertical augmentation Augmentation of bone height, e.g., with bone block grafts from the patient's own jaw.
  • Sinus lift Special method for the posterior maxilla – more on this in the next section.

Sinus lift – bone grafting in the upper jaw

The Sinus lift is one of the most common augmentations of all. Reason: In the posterior maxilla, the maxillary sinus is located directly above the tooth roots. After tooth loss, the bone there often reduces so much that only a few millimeters of bone remain between the alveolar crest and the maxillary sinus – too little for an implant.

During a sinus lift, the mucous membrane of the maxillary sinus is gently detached from the bone and slightly elevated. Bone graft material is then placed into the resulting cavity, which transforms into the patient's own bone over the healing period. After several months, sufficient stable bone is available to securely place an implant.

Internal vs. external sinus lift

If the existing bone is only slightly reduced (at least 4-5 mm in height), often a internal sinus lift – a minimally invasive procedure performed directly during implantation. If bone loss is greater, a external sinus lift necessary, with access via a small opening in the maxillary sinus wall.

What bone graft materials are used?

In modern dentistry, various scientifically well-documented materials are available. At Denis-Focus Dental, we individually decide which material is best suited for your situation:

  • Autologous bone The gold standard, taken from another jaw region. Very good osseointegration, only necessary for larger defects.
  • Animal bone (xenogeneic): Specially prepared bovine material that serves as a scaffold and is slowly replaced by the body's own bone. Advantage: no second surgery.
  • Synthetic bone graft substitute (alloplastic): High-purity, ceramic-based materials like beta-tricalcium phosphate. Biocompatible and well-tolerated.
  • Membrane technology A resorbable membrane protects the graft material during healing and prevents soft tissue ingrowth.

Process and Healing

The procedure is performed under local anesthesia and takes between 30 and 90 minutes, depending on the scope. Most patients report that the procedure was significantly less stressful than expected. After the operation, mild swelling and a feeling of pressure may occur in the first 2-3 days – cooling and common painkillers reliably help.

The healing time depends on the extent of the bone augmentation: small augmentations require about 3 months, while larger procedures such as an external sinus lift can take up to 6 months. During this time, the replacement material integrates with the patient's own bone. Only after this period is the implant placed – or, if the initial conditions are favorable, at the same time as the bone augmentation.

Good to know

Costs and Planning

The cost of a Bone augmentation depend heavily on the extent of the procedure. A small local augmentation performed as part of an implant procedure is significantly less expensive than a major sinus lift or a bone block graft. Frequently Asked Questions about Bone Augmentation and Sinus Lifts

A Bone augmentation (Augmentation) is a dental procedure in which missing jawbone is rebuilt using the patient's own bone material or high-quality bone graft substitutes. It creates the foundation for the secure placement of a dental implant.
A Sinus lift is a special form of bone augmentation in the posterior upper jaw. Here, the floor of the maxillary sinus is gently lifted, and the resulting space is filled with bone substitute material to create sufficient bone for a dental implant.
A bone graft becomes necessary when the jawbone has receded so much due to tooth loss, inflammation, or long-term denture wear that it doesn't provide sufficient support for an implant. In many cases, the bone graft can be performed at the same time as the implant placement.
The procedure is performed under local anesthesia and is largely pain-free. After the treatment, mild swelling and tenderness may occur, which can be well managed with common painkillers. Most patients can resume their daily activities the very next day.
